Good Standing/Disciplinary History

All applicants are required to submit evidence of Good Standing and both public and private Disciplinary History.  Below please find a template for requesting the requisite information from your state.  Please note that some states require their own request form and/or fee.

Educational Experience (MCLE)

All applicants are required to submit Educational Experience (CLE).  Below please find the CLE History Form.  Applicants must provide 36 relevant hours in the 3 years preceding application (45 hours - CA only).  Please note if  you are applying for a QIC Application Fee Waiver - CLE must be submitted for NACC to award you a waiver.

Please upload your Resume/CV and Writing Sample (drafted within the last 3 years) below.  Both are required of all applicants.  If you are applying for a Substantial Involvement Waiver, please submit a detailed summary of your work as a judge, supervisor, policy advocate, etc.
